CHECK LIST FOR POLISHNATIONAL VISA –WORK POLAND

THEAPPLICATIONMATERIALSHOULDBE PLACEDINTHEFOLLOWING ORDERREQUIREDDOCUMENTS:

1.PASSPORT-shouldhavebeenissuedwithintheprevious10years;itsvalidityshouldextendatleast3monthsaftertheintendeddateofdeparturefromtheSchengen territory;musthaveatleast twoblankpages.

2.VISAAPPLICATION

2.1Visaapplicationformwithoutblanks,signedtwicebytheapplicant-atleast18yearsoldmaylodgeandsignavisaapplication personally:thesignatureasthe oneinthepassport.

2.2.Two colorphotos -specifications: passporttype, whitebackground35mmx45mm, nomorethan6-month-old.

3.FLIGHTITINERARY(RESERVATIONONLY)

  1. TRAVEL MEDICAL INSURANCE (TMI) – one copy of the certificate of TMI issued by a company from the list of approved Indianinsurance companies which meet the necessary conditions referred to in Article 25 (1) (2) (a) and Article 25 (1b) of the Act of 12December2013onForeigners-availableathttps://www.gov.pl/web/diplomacy/visas(handwrittencertificateswillnotbeaccepted; the name of the insured should be written in Latin alphabet; TMI must be valid in the Schengen area and for the entireduration of stay; the TMI’s minimum coverage is 30.000 euro and it must cover costs of any emergency medical treatment andrepatriationformedical reasons as well asincaseofdeathrepatriationofthedeceased);

6.DOCUMENTSCONFIRMING THE PURPOSEOF THE TRIP

6.1.Originalandvalidworkpermitand itscopy.

Requirements regarding the costs of living and return travel to the country of origin or residence contained in The Regulation ofthe Minister of the Interior of Poland of 23 February 2015 on financial resources required from a foreigner entering the territory oftheRepublicofPolandanddocumentsthatmayconfirmthepossibilityofobtainingsuchfunds,aswellasthepurposeandduration.

6.2.Proofofregistration oftheemploying company. (KRS–National CourtRegisterof Poland)

6.3.Originalemploymentletterfromtheapplicant’semployerinPolandontheofficialcompanypaperwithstampandsignature,date and clearly mentioning full address and contact details of the company, the name and position of the countersigning officer,detailsregardingtheemploymentoftheapplicant(e.g.,position,salary,estimatedduration),detailsregardingadditionalconditionsofemployment(e.g.,accommodation,transport,livingexpenses).

6.4.Incaseof internationaltransfer of the employee: a letterfrom the current employer confirmingthetransfer and approvingtheleave/absenceoftheapplicant.

6.5.Supportingdocuments(optional):

6.5.1.Professional/educationalcertificatesprovingprofessionalskills-copiesandoriginalslegalized/attested/apostilledbyMEA/MFA.

6.5.2.Workexperiencecertificate-copiesandoriginals.

6.5.3.PoliceClearanceCertificate.

  1. ACCOMODATION – documentconfirmingthebooking of appropriateaccommodationfortheintendedperiodofstay inPoland:leaseagreementorothertype ofaccommodation proof.

8.ADDITIONALDOCUMENTS-applicantsareallowedtosubmitadditionaldocumentsthattheyconsiderusefultoexplain/justifythetrip.

9.RESIDENCE PROOF –anIDconfirming legalresidencewithintheEmbassy’s/ Consulate’sjurisdiction.

10.PASSPORT DATAPAGESCOPY

10.1.Onecopy oftheapplicant’spassportdatapages(thefirstandthelastone).

10.2.CopyofthepageswithPolish/Schengenvisas,if issued- ifthe visaswereinthepreviouspassport,pleaseattachcopy ofthatpassport’sdatapages-thefirstandthelastone.
